ACUVUE® Go Dailies Programme
TERMS AND CONDITIONS
- ACUVUE® Go Dailies Programme is organised by ACUVUE® Malaysia (“Organiser”). By participating and entering in this Promotion, you (“Participant”) agree to be bound by the Terms and Conditions set out herein.
- The Promotion is available from 7th July 2020 to 30th September 2020 (“Promotion Period”). The Organiser reserves the right to amend the Promotion Period at any time without any prior notice.
- The Promotion is open to all Malaysians aged 16 years and above as of 1st January 2020 who reside in Malaysia.
- Participants are eligible to receive one (1) unit of GrabFood e-voucher worth RM20 and one (1) eye health examination (“Promotion”) if they fulfil the following criteria:
a) Register the following details onto www.acuvue.com.my/godailies
- Full name
- Mobile No.
- Date of birth (Month and Year only)
- Eye Care Professional of Choice
- Experience with contact lenses (ACUVUE® user/Non-ACUVUE® user/Non wearer)
- Check box to signify acceptance of Terms & Conditions and consent to use of data
b) Purchase 2 boxes of any of the following two products at participating outlets
- ACUVUE® OASYS 1-DAY
- ACUVUE® DEFINE 1-DAY
- ACUVUE® MOIST 1-DAY
- ACUVUE® MOIST for ASTIGMATISM 1-DAY
- ACUVUE® MOIST MULTIFOCAL 1-DAY
c) Sends proof of purchase and redemption details to www.acuvue.com.my/godailiesredemption
- Mobile No.
- Upload Proof of Purchase
- Eye Care Professional visited
- Products purchased
- Check box to signify acceptance of Terms & Conditions and consent to use of data
- Purchase transaction and submission of redemption details must be done within the Programme Period in order to qualify for the redemption.
- Eye health examination includes subjective refraction and corneal check. If the Participant is new to contact lenses, he/she will be provided contact lens fitting and aftercare.
- The process of eye health examination may vary by outlet.
- Each Participant is limited to maximum of 4 redemptions for this Promotion. The Promotion is not transferable including the Eye Health examination.
- Redemption of Eye Health examination is valid for 3 months from the date of purchase. Any extension is up to the discretion of the eye care professional.
- The GrabFood e-voucher of RM20 will be sent to the Participant’s WhatsApp which is as per the registered mobile number at www.acuvue.com.my/godailiesredemption. If the registered mobile number does not have a valid WhatsApp account and a Grab account, the submission is automatically voided.
- Please allow up to 2 working days for the e-voucher to be sent over via WhatsApp.
- GrabFood e-voucher is not exchangeable for any other form of redemption unless stipulated by the Organizer.
- The Organiser, all its promotion partners, and other parties involved in the Promotion, including their directors, officers, and agents are not liable in any way for the following: (a) intervention by unauthorised parties during the Promotion; (b) any human errors and electronic/technical failure in the administration of the Promotion and processing of entries; and (c) any liability (including death) whether directly or indirectly caused by their participation in the Promotion.
- The Organiser at its sole and absolute discretion, reserves the right to reject any redemption that is wrong, incomplete, unclear, late, or with wrong or insufficient proof of purchase. Any redemption containing elements of allegations or defamation, involving the misrepresentation, infringing (or may be infringing) any copyright, trademark, or any intellectual property rights or proprietary rights of any entity, both for survival and for the deceased (including privacy rights, publicity, or fraudulent images) will be disqualified.
- All expenses incurred to participate in this Promotion are the sole responsibility of the Participant.
- By participating, accepting, and/or using any of the redemption item(s), the Participant agrees to release liability of the Organiser, all its business partners, and other parties relating to this Promotion, including their directors, officers, employees, and agents from any claim, prosecution, judgment, damage, or loss.
- The Organiser shall not be liable for any default or loss suffered by any party due to natural disaster, war, riot, strike, industrial action, epidemic, fire, flood, storm, technical failure, or any event that is beyond the reasonable control of the Organiser.
- By participating in the Promotion, the Participant has agreed to be bound by the Terms and Conditions and agreed that all matters relating to this Promotion are final and binding on all Participants of this Promotion, and no correspondence will be entertained.
- The Organiser may, at its sole and absolute discretion, amend the Terms and Conditions set herein without any prior notice.
- The Organiser reserves the right to modify, cancel, terminate, or suspend this Promotion with a notification on https://www.facebook.com/AcuvueMalaysia or in any other manner. The Participant shall not make any claim or compensation from the Organiser for any losses incurred.
- To the maximum extent permitted by law, the Participant agrees to waive any right against the Organiser and/or officers, employees, representatives, and/or designated agents (including suppliers or any third party) in respect of loss or damage arising from the use of the redemption item or from the participation in this Promotion.
- By participating in this Promotion, each Participant consents to the Organiser’s collection, use and processing of the personal data provided by the Participant, which includes the Participant’s name, email address, mobile number, date of birth, experience with contact lens and any information which may identify the Participant (collectively, “Personal Data”) for the purposes relating to the Promotion as specified in these Terms and Conditions (“Purpose”).The Organizer collects your Personal Data to enable administration of this Promotion. Your Personal Data may be provided to third parties assisting in the conduct of the Promotion, including our service provider, suppliers and deliverers. If the Personal Data requested is not provided, you may not participate in this Promotion.
- By participating in the Promotion, you also consent to the collection and use of your Personal Data by the Organizer for future marketing and promotion purposes. The Participant has the right to obtain access, request correction, or withdraw consent to disclose any personal information held by the Organiser. Such requests can be made via email to the Organiser at [email protected].
